I understand that the O.M.E.G.A. Academy mentors will serve as advisors, guides, adult friends, and supporters for me throughout my participation in this program. They will assist me in my personal, social, and academic development. I promise to work hard on my personal development and agree to participate fully in the program. I will strive to always be in attendance and on-time for meetings and field trips, and to abide by all rules, regulations, and guidelines. I understand the goals of the O.M.E.G.A. Academy and enter into this Covenant Agreement willingly.

I understand and subscribe fully to the mission and goals of the O.M.E.G.A. Academy. I know that the mentors will not attempt to serve as surrogate parents, but will assist my child by providing support to their personal, social and academic development. I agree to provide whatever support I can that will aid my child in becoming his best. I further agree to hold harmless the Omega Psi Phi Fraternity, Inc., the O.M.E.G.A. Academy mentors, and any volunteer affiliates for any injury or loss that may occur as a result of my child’s participation in this program.
